The fourth volume of the 1% Lifesteal series opens with Freddy Stern newly responsible for the city of Repentawa after its former rulers are killed or driven out. He takes control of essential businesses to stabilize the collapsing economy, oversees the punishment of collaborators, and works with his companion Sophia to fund the city's recovery, including through the sale of his own regenerating organs. As Freddy struggles with the psychological aftermath of a recent extended isolation ordeal, he and Sophia detect a growing, unexplained threat linked to madness spreading somewhere beyond the city. When imperial authorities confirm that a transformed former rival is gathering an infected monster horde that could destroy the region, Freddy commits Repentawa's defenses and his own abilities to preparing for the coming attack.
