In 1984 Tokyo, assassin Aomame enters a subtly altered reality with two moons, which she calls 1Q84. Tengo, a mathematics teacher and aspiring novelist, secretly rewrites teenager Fuka-Eri's Air Chrysalis, whose Little People and two moons prove to describe that reality. Aomame and Tengo gradually realize that each is searching for the other after a single moment of childhood intimacy. Aomame kills the abusive, supernatural Leader of the Sakigake commune, then hides from his followers and discovers she is pregnant despite having had no recent sexual contact. Tengo cares for his dying father while moving closer to Aomame through visions, writing, and Fuka-Eri's guidance. Investigator Ushikawa tracks both of them but is killed by Tamaru before he can deliver Aomame to Sakigake. Aomame and Tengo reunite, affirm their love, and escape by the expressway stair into a world with one moon. Its small differences leave their exact reality uncertain, but they face it together and intend to protect their unborn child.

In 1984, Aomame leaves a taxi trapped on Tokyo's elevated expressway and descends an emergency staircase so she can reach an appointment. The taxi driver's warning that unusual actions can change what one sees begins to feel literal when she notices that police uniforms and equipment differ from what she remembers. Aomame is a fitness instructor and a precise killer working for the wealthy Dowager, who shelters abused women and targets men responsible for severe violence. In a parallel story, cram-school mathematics teacher Tengo Kawana is asked by his editor, Komatsu, to rewrite Air Chrysalis, a striking manuscript with poor prose. Its seventeen-year-old author, Fuka-Eri, accepts the plan with unsettling calm. Her story describes mysterious Little People and an object woven from threads drawn from the air. Tengo agrees to the deception, attracted by the manuscript's imaginative force despite the risks.

Tengo learns from Professor Ebisuno, Fuka-Eri's guardian, that she grew up in Sakigake, a secluded religious commune led by her father. She escaped but left another girl behind, and Ebisuno hopes publication will draw attention to the community. Tengo transforms Air Chrysalis while preserving its strange world of Little People, a two-moon sky, and paired beings called maza and dohta. The book wins a prize and becomes a sensation, but Fuka-Eri's public visibility creates new danger. Aomame befriends policewoman Ayumi and continues the Dowager's work. She recognizes that news and memories no longer align, then sees a second, smaller moon beside the familiar one. Concluding that she has crossed into a different version of the year, she privately names it 1Q84, with the question mark standing for everything she cannot explain.

Air Chrysalis's success intensifies the connection between fiction and the altered world. Fuka-Eri stays with Tengo, and her presence seems to draw him more deeply into the reality her story describes. Both Tengo and Aomame remember attending the same elementary school. Aomame was isolated by her family's strict religious life, while Tengo was burdened by his father's collection rounds. One day she took Tengo's hand and held it without speaking; neither has forgotten the moment, though they have not met since childhood. Aomame learns that Ayumi has been murdered, deepening her isolation. Ushikawa, an investigator with extensive knowledge of Tengo's life, approaches him through an offer of foundation support and asks questions that suggest powerful people are monitoring Air Chrysalis and its author. The Dowager, meanwhile, prepares Aomame for a far more dangerous target connected to Sakigake.

The Dowager asks Aomame to kill Sakigake's Leader, who is accused of ritual sexual abuse involving young girls. Aomame enters his guarded hotel room posing as a therapist. Leader knows why she has come and says that his body has become a receiver for forces represented by the Little People. He describes Fuka-Eri as a separated counterpart and claims that his death is necessary to restore balance. He also knows that Aomame loves Tengo and warns that killing him will place her beyond an easy escape. Aomame carries out the painless technique she uses on abusers. Sakigake immediately begins searching for her, and Tamaru moves her into a concealed apartment prepared by the Dowager. Aomame accepts hiding as the cost of the killing but continues to search the city for some sign of Tengo.

Tengo visits his estranged father, now losing consciousness in a seaside sanatorium, and tries to understand the uncertain story of his own parents. The boundary between Air Chrysalis and his life grows thinner: Fuka-Eri's strange influence, the two moons, and visions associated with an air chrysalis all point toward Aomame. From her hideout, Aomame watches a nearby playground and unexpectedly sees Tengo there looking at the moons. Sakigake's pursuit prevents her from approaching him safely, but the sight confirms that he is in the same altered world. Believing for a time that protecting him may require her death, she instead finds a new reason to live when she realizes that she is pregnant despite having had no recent sexual relationship. She understands the pregnancy as a connection to Tengo. The two remain physically separated at the close of this stage, but each now knows that the other is near and still remembers their childhood bond.

Ushikawa begins a methodical investigation of Aomame, Tengo, Fuka-Eri, the Dowager, and Sakigake. Acting for the commune but aware that failure may cost him his life, he reconstructs the childhood link between Aomame and Tengo and watches Tengo's apartment building. Aomame remains hidden, exercising, reading reports, and protecting her pregnancy while Tamaru supplies her and maintains strict security. She decides that reunion with Tengo matters more than indefinite concealment. Tengo divides his time between Tokyo and his father's sanatorium, where nurse Kumi Adachi befriends him. His father's coma and the recurring image of an air chrysalis force Tengo to consider that his mother, his uncertain parentage, and the present supernatural events may be connected. Fuka-Eri remains close but continues to act according to purposes she does not fully explain.

Ushikawa's surveillance brings him steadily closer to the hidden relationship at the center of events. He photographs Fuka-Eri and Tengo and identifies the playground as a significant location, while Sakigake presses him for results. Tengo's father dies, leaving Tengo with no simple answer about his birth but loosening the hold of their painful history. Tengo writes a new story of his own and accepts that Aomame is not merely a memory or vision. Aomame learns through Tamaru that someone has been watching Tengo's building and understands that the search is closing in. She nevertheless resolves to reach Tengo and leave 1Q84 rather than surrender their possible life together. The competing searches converge: Ushikawa seeks Aomame for Sakigake, Tamaru seeks the watcher threatening her, and Tengo and Aomame independently move toward the place where they saw one another beneath the two moons.

Tamaru captures Ushikawa before he can report Aomame's location to Sakigake and kills him, protecting the Dowager's network and delaying the commune's pursuit. Fuka-Eri leaves Tengo after guiding him toward an understanding of Aomame and the altered world. Tengo goes to the playground, where Aomame finally approaches him. They recognize that the childhood moment when she held his hand has shaped both their lives, and they choose one another without further delay. Aomame explains that she is pregnant and believes Tengo is the child's father through the strange transfer that occurred while they were apart. She leads him to the expressway and finds the emergency stair by which she entered 1Q84. At the top they emerge into a version of Tokyo with only one moon. A changed sign keeps them from knowing whether this is precisely their original 1984, but the immediate threat has receded. They face the uncertain world as a couple, committed to protecting their unborn child.

Ushikawa traces Tengo to the apartment building from which he has been watching Aomame's refuge. Tamaru captures and kills him, then arranges the body's discovery without exposing the Dowager's network. Fuka-Eri leaves Tengo after helping him understand where Aomame is, while Sakigake's surviving operatives continue to seek the person who killed their Leader. Tengo and Aomame finally meet in the playground where each had separately watched the two moons. They confirm that neither forgot the moment they held hands as children. Aomame tells Tengo that she is pregnant and believes the child is his, conceived through the strange connection that joined them even while they were apart. She leads him back to the Metropolitan Expressway emergency stair. They climb into a world that appears to be 1984 rather than 1Q84: only one moon is visible, although a changed highway sign suggests that their destination may not be identical to the past they left. They choose to remain together and protect the child, accepting uncertainty about the world around them.
