CERN scientist Leonardo Vetra is murdered and a canister of antimatter is stolen, leading symbologist Robert Langdon and Vetra's daughter Vittoria to Vatican City during a papal conclave. A caller claiming to represent the Illuminati threatens to kill four leading cardinals along an ancient Path of Illumination, then destroy the Vatican at midnight. Langdon and Vittoria follow clues through Roman churches but cannot prevent the ritual deaths. Camerlengo Carlo Ventresca appears to locate the antimatter through divine inspiration and carries it aloft in a helicopter before it explodes, saving the city. Langdon survives the blast, and evidence left by CERN director Maximilian Kohler reveals that Ventresca engineered the entire conspiracy. He had killed the pope and used the Illuminati legend to create a miracle that would renew the Church. After learning he was secretly the pope's son, Ventresca kills himself. Cardinal Mortati becomes pope, and Langdon and Vittoria leave Rome together.

Harvard symbologist Robert Langdon is summoned to CERN in Switzerland by its director, Maximilian Kohler. Physicist and priest Leonardo Vetra has been murdered, his eye removed and his chest branded with a symmetrical word marking associated with the Illuminati, a historical enemy of the Catholic Church that Langdon believed extinct. Vetra's adopted daughter and research partner, Vittoria, returns to explain their secret experiment: they created antimatter in an attempt to study creation itself. A portable canister containing the substance has been stolen. Its magnetic trap will fail when its battery expires, allowing the antimatter to meet ordinary matter in a catastrophic explosion. Security video shows the canister somewhere inside Vatican City, where the College of Cardinals is gathering to elect a pope after the previous pontiff's death. Langdon and Vittoria leave for Rome to warn the Vatican.

At Vatican City, the Swiss Guard initially resists the outsiders' claims, but the missing antimatter canister and a threatening telephone call establish the danger. Four preferiti, the cardinals considered the leading papal candidates, have vanished before the conclave. The caller claims the Illuminati will murder one cardinal at each hour on a sequence of altars representing earth, air, fire, and water, then allow the antimatter to destroy the Vatican at midnight. Camerlengo Carlo Ventresca, the late pope's aide and the temporary administrator of the Holy See, chooses to keep the conclave in place while the Guard searches. Langdon concludes that a seventeenth-century trail known as the Path of Illumination may identify the four sites. A clue in Galileo's writings directs him and Vittoria toward Rome's churches and monuments.

Langdon and Vittoria trace the first marker to the Chigi Chapel in Santa Maria del Popolo, whose sculpture and design point onward along the supposed path. They arrive too late to save the first missing cardinal, who has been branded with the symbol for earth and killed in a manner matching it. The killing confirms that the threat is being staged as an Illuminati ritual. BBC reporter Gunther Glick and camera operator Chinita Macri, acting on an anonymous tip, begin reporting the crisis and bring crowds and worldwide attention to Rome. Langdon uses the direction indicated by Bernini's sculpture to identify the next location, while Vittoria and the Swiss Guard struggle to work within a timetable set by the killer.

The next clue leads to St Peter's Square, where the second cardinal has been fatally wounded and marked for air. Langdon attempts to revive him but cannot. The search continues through Bernini's works toward Santa Maria della Vittoria. There the assassin has suspended the third cardinal above a fire and branded him with the corresponding element. Commander Olivetti is killed in the confrontation, and Langdon is trapped beneath a heavy sarcophagus lid before Vittoria helps free him. The assassin escapes with Vittoria as his captive. With two senior security figures gone and time running out, Langdon must pursue both the Path of Illumination and the abducted Vittoria while the Vatican still has no location for the antimatter.

Langdon reaches the fourth altar at the Fountain of the Four Rivers in Piazza Navona, where the last preferito has been weighted down and thrown into the water. He dives in but cannot save the cardinal. The monument's design points him toward Castel Sant'Angelo, the suspected Illuminati lair. Inside, Langdon finds and frees Vittoria, and together they defeat the assassin, who falls to his death. They discover a concealed route linking the fortress to the Vatican and hurry back. Meanwhile Maximilian Kohler has arrived with information about the conspiracy and enters the camerlengo's office. The encounter ends with Kohler shot by the Swiss Guard, but before dying he gives Langdon a small video camera and urges him to reveal what it contains.

After the camerlengo emerges wounded, he claims to receive guidance showing him where the antimatter has been hidden. He finds the canister in the tomb beneath St Peter's, takes it into a helicopter, and flies high over the city. Langdon boards at the last moment. The camerlengo parachutes away, leaving Langdon to improvise an escape with a protective cover as the antimatter explodes harmlessly overhead. Both men survive, and the camerlengo is acclaimed as a miraculous saviour. Langdon then watches Kohler's recording and learns that Ventresca himself planned the murders and the theft, using the supposed Illuminati attack to revive faith and position himself as the Church's hero. Ventresca had also killed the late pope after discovering he had fathered a child. Cardinal Mortati reveals that the child was conceived by artificial insemination and was Ventresca himself. Confronted with the truth, Ventresca burns himself to death. Mortati is elected pope, Vittoria and Langdon leave together, and the surviving Illuminati brand is entrusted to Langdon.

Langdon and Vittoria discover that the supposed Illuminati plot was orchestrated by Camerlengo Carlo Ventresca. He murdered the pope after learning that the pontiff had fathered a child through artificial insemination, then arranged Leonardo Vetra's death, the antimatter theft, and the cardinals' murders to manufacture a crisis in which he could appear divinely chosen. Kohler had recorded Ventresca's confession, and the surviving evidence reaches the conclave. Ventresca learns that he himself was the pope's child, conceived without breaking the pope's vow of celibacy, and publicly burns himself to death. Cardinal Mortati is elected pope. The antimatter has exploded harmlessly high above Rome after Langdon survives the fall from the helicopter with a parachute-like cover. Vittoria and Langdon leave the Vatican together, and Mortati gives Langdon the final Illuminati diamond brand on indefinite loan.
