Previously, in Elena's story: Elena Michaels, the only known female werewolf, abandoned an attempted human life after rogue werewolves attacked Jeremy Danvers's Pack and forced her hidden worlds together. She chose Stonehaven, resumed her relationship with Clay, and accepted the Pack as her family. Later, human researchers captured Elena with other supernatural people and experimented on them before Clay, Jeremy, and an alliance of witches, vampires, and half-demons destroyed the compound. Elena returned knowing that werewolves are part of a much larger concealed population. She and Clay now live as mates within Jeremy's Pack, carrying the losses of the Pack members killed during Elena's return but no longer divided over where she belongs.

Elena is pregnant, something no known female werewolf has experienced, and neither Pack history nor human medicine can fully answer what will happen. She and Clay are pleased but cautious, particularly about whether she can Change safely. Xavier Reese, a half-demon able to teleport, approaches the Pack with a bargain. He wants Elena and Clay to steal a valuable letter associated with Jack the Ripper from the protected collection of sorcerer Patrick Shanahan. Dense fog appears where it should not, rats associated with old disease infest the area, and dead bodies do not remain inert. Elena and Clay realize that returning the document will not necessarily undo what has happened. Their first priority is to contain incidents before human police or reporters connect them, but each manifestation expands the area they must watch. Xavier knows more about the object's value than he initially disclosed, yet he also lacks a straightforward way to close the passage. Elena continues the investigation while monitoring the pregnancy and resisting Clay's instinct to move her away from every physical risk.

The Pack concludes that the letter has become an anchor for a passage between present-day Toronto and the London of the Ripper murders. The breach allows more than visions to cross: rats, corpses, and pieces of the older city's atmosphere can become physically dangerous. Elena and Clay seek local supernatural knowledge and meet Zoe Takano, a vampire familiar with Toronto's hidden population. She has her own reasons to prevent a public outbreak and helps them separate rumors about the letter from the effects now occurring. The group considers whether the manifestations follow the Ripper history or whether someone in the present is directing them. Elena's heightened concern for the unborn child makes each encounter harder, but it also sharpens her refusal to leave a threat that could follow the Pack home.

Plague-bearing rats and animated dead make simple containment impossible, and the pattern suggests that a person with specialized knowledge is exploiting the breach. Research into Victorian London brings the investigators to Hull, who presents himself as someone able to explain the portal and the history attached to the letter. His information is useful but incomplete. The group cannot determine whether he is trying to close the opening, control it, or steer them toward a particular solution. The immediate supernatural outbreak is contained without revealing werewolves to the human city. Elena has endured the investigation, several transformations, and medical uncertainty without losing the pregnancy. At the close she and Clay learn that they are expecting twins. They return to Stonehaven with the danger over, the Pack preparing for an unprecedented addition to its family, and Elena no longer treating pregnancy as a reason to withdraw from the life she chose.
