After station chief John Strangways and his secretary disappear in Jamaica, Bond is sent to investigate. The trail leads to Crab Key, the private guano island of Dr Julius No, who is suspected of killing intruders and interfering with American missile tests. Bond and his old ally Quarrel land secretly and meet shell collector Honey Rider. Dr No's armoured flame-throwing vehicle kills Quarrel, and Bond and Honey are captured. No reveals that he works with Soviet interests and intends to dispose of them through elaborate ordeals. Bond survives an obstacle course and a fight with a giant squid; Honey escapes a separate attempt on her life. Bond commandeers machinery and buries Dr No in guano, killing him. He and Honey flee the island and return to Jamaica, leaving the authorities to secure the installation.

In Jamaica, station chief John Strangways and his secretary Mary Trueblood are killed and removed by an organised group, leaving their office to appear suddenly abandoned. In London, Bond has recovered from poisoning sustained on his previous mission. M questions his recent judgement, orders him to adopt a different handgun, and sends him to determine what happened in Jamaica. The assignment is presented as a way to return to work without excessive danger. Bond arrives knowing only that Strangways had become interested in Crab Key, a privately owned island whose protected bird colony and guano business discourage visitors.

Bond consults colonial official Pleydell-Smith and discovers missing records and widespread unease around Crab Key's owner, Dr Julius No. Strangways had linked the island to unexplained disruption of American missile trials and to the deaths of bird wardens. Attempts are made to frighten or kill Bond, including a poisoned delivery and a venomous centipede placed in his bed. He recruits his old ally Quarrel, who knows the local sea and has helped Strangways. They train and prepare a secret night crossing to Crab Key, accepting that official channels have been watched or compromised.

Bond and Quarrel reach Crab Key by small boat and conceal their landing. On the beach they meet Honeychile Rider, an independent shell collector who visits the forbidden shore for rare specimens. Bond persuades her that the island's guards are now actively hunting them. The three move inland through marsh and cover while patrols search the coast. Honey's knowledge of the terrain helps them avoid immediate capture, but Dr No's organisation deploys a vehicle disguised as a fantastic creature to terrify trespassers and drive them from hiding.

The disguised vehicle is armoured and equipped with a flame weapon. It kills Quarrel and captures Bond and Honey. They are taken into Dr No's highly organised installation, cleaned, fed, and treated with artificial courtesy while remaining prisoners. The comfort is another means of control, and staff monitor them closely. Bond searches for weaknesses and tries to preserve his strength while Honey resists the institution's intimidation. They are brought before Dr No, who has built a private domain around the island's guano industry and its strategic position near American weapons testing.

Dr No describes his criminal history, his physical injuries, and his work interfering with American guided-missile tests on behalf of Soviet interests. He treats Bond and Honey as subjects for demonstrations of endurance rather than as prisoners who might leave alive. Bond is placed in a constructed obstacle course that exposes him to heat, electrical shocks, dangerous creatures, and exhaustion. He understands that the route is designed both to gather observations and to kill him while satisfying No's curiosity. Honey is taken away for a separate staged death, leaving each unaware whether the other survives.

Bond completes the obstacle course and survives a final underwater struggle with a giant squid. Honey also escapes: the crabs meant to consume her do not behave as Dr No predicted. Bond enters the island's industrial area, takes control of a loading machine, and sends Dr No into the guano flow, where he is buried and killed. He finds Honey, and they use the armoured vehicle to cross the island before escaping by boat. Back within reach of Jamaica, Bond prepares a report and instructions for authorities to secure Crab Key and its communications equipment. Quarrel is dead, but No's operation is destroyed, and Bond and Honey recover together.

Dr No explains that he is disrupting American missile tests for the Soviet Union and intends to kill his captives through staged ordeals. Bond is forced through a hazardous obstacle route built into the installation and survives a final struggle with a giant squid. Honey, left to be eaten by crabs, escapes because the animals do not attack as Dr No expected. Bond reaches the working area, takes control of a loading machine, and sends Dr No into the flow of guano, where he is buried and killed. Bond reunites with Honey, and they escape Crab Key in the armoured vehicle used to frighten intruders before sailing back toward Jamaica. Bond arranges for the authorities to take control of the island and frames the immediate report to protect the mission's sensitive elements. He and Honey recover together after the operation.
