The alien Borant Corporation collapses every building on Earth, kills most of humanity, and turns the planet into an eighteen-floor dungeon broadcast across the galaxy as entertainment. Carl, caught outside in his boxer shorts chasing his ex-girlfriend's show cat, enters the dungeon with the cat, Princess Donut, who gains human intelligence from an early reward and becomes a talking spellcaster who insists on leading their party. Guided by Mordecai, a gruff tutorial-guild manager, they fight through the first floor's goblins and bosses, ally with the elderly survivors of the Meadow Lark care facility, and become breakout stars of the broadcast. On the second floor Carl kills a devastating Rage Elemental with a bomb-rigged vehicle in front of the whole universe, the pair appear on Odette's intergalactic talk show, Carl makes an enemy of the Skull Empire's Prince Maestro while rescuing trapped crawlers on a rival program, and Donut tames a pet dinosaur, Mongo. The book closes with the trio descending to the third floor, where every surviving crawler must choose a new race and class.

The alien Borant Corporation instantly collapses every building on Earth, killing everyone inside, and announces that survivors may reclaim their planet by conquering an eighteen-floor World Dungeon whose entrances stay open only briefly. Carl, outside in boxer shorts and his ex-girlfriend's Crocs to retrieve her show cat Princess Donut, has no other way to survive the cold and enters the dungeon with the cat. On the first floor he kills a goblin to earn shelter in a tutorial guild run by Mordecai, who becomes their guide, and completes the tutorial, learning that roughly thirteen million humans have entered and that the crawl is a galactic broadcast run for an audience. A reward from an early loot box transforms Donut: she gains human-level intelligence and speech, and with stats higher than Carl's she formally becomes party leader of the self-declared Royal Court of Princess Donut. Carl, still without shoes or pants, starts to accumulate the fighting skills, oddball achievements, and explosive habits that will define him.

Carl and Donut settle into the rhythm of the first floor: grinding low-level monsters, sheltering in safe rooms staffed by non-player attendants, and learning the dungeon's economy of loot boxes, achievements, and fan attention. They defeat their first boss, an encounter that leaves Carl shaken by how the game dresses its victims. The first broadcast recap episode airs, showing the pair glimpses of other notable crawlers, including the unsettling child prodigy Lucia Mar. They find evidence of crawlers murdering crawlers, tied to a man named Frank Q. Donut discovers the chat system, and her charisma, together with Carl's goblin-friendly tattoo, lets the pair strike a bargain with two goblin engineers, trading salvage for a ramshackle vehicle instead of fighting.

The goblin alliance ends in fire: Carl loots the goblin workshop and destroys it, an act the System memorializes with achievements he is not proud of. The pair earn passes to the Desperado Club, an underground establishment for crawlers, and survive their first direct brush with the player-killers Frank Q and Maggie My, escaping an attempted ambush in a safe room. Riding their goblin-built vehicle east for hours, they follow spray-painted signs left by other survivors and find the people who made them: a band from the Meadow Lark elder-care facility, led by the caregiver Imani with the Andrews brothers and dozens of elderly residents in tow. The two groups exchange stories and begin planning to reach the floor's exit together.

Allied with the Meadow Lark group, Carl and Donut grind the surrounding tunnels, conceal the group's trail, and take down the Juicer, a boss guarding the neighborhood. After a stop at a training guild to learn new spells and finally armor Carl's hands, the combined force fights the area's true gatekeeper, a massive boss called the Ball of Swine, and defeats it using a fortified redoubt. Now level ten, Carl and Donut agree to keep helping Meadow Lark's residents reach safety. As the pair descend toward the second floor they are teleported out of the dungeon to a production trailer: their growing fame has earned them a booking on Dungeon Crawler After Hours, the intergalactic talk show hosted by Odette.

On Odette's show Donut charms the galaxy while Carl studies how the game is packaged for its audience; afterward Odette talks candidly with them about the crawl, the coming race selection, and the fact that random loot is not truly random. On the second floor the pair fight new monsters, and Carl helps the elderly Elle McGibbons to safety as the broadcast warns that Rage Elementals will be released on the floor. Carl weaponizes his growing crafting skill, building his first signature bombs. When the pair try to skip a boss chamber, the boss, a clone of a monster called the Krakaren, comes to them: Carl kills it with a massive explosion and nearly dies shielding Donut with his own body, and Donut, dragging him to safety, admits how much he means to her.

Recovering, the pair find Mordecai transformed into a new body, meet their first Borant dungeon administrator, and gain a public-relations liaison, Zev, who manages their growing celebrity. They rejoin the Meadow Lark group for the push to the stairs, but disaster strikes: a resident's harmless accident triggers the release of a Rage Elemental as punishment, and the creature kills Yolanda and another member of the group before the survivors reach shelter. With the game's rules newly changed to stop crawlers from waiting out the floor in safe rooms, Carl builds a bomb-rigged vehicle and, with the entire universe watching, leads the Rage Elemental on a wild chase and destroys it through a chain of bombs, obstacles, and a rules exploit that becomes an instant broadcast legend.

The Meadow Lark survivors descend to the third floor ahead of them while Carl and Donut finish the second. Pulled onto the sadistic rival show Death Watch Extreme Dungeon Mayhem, Carl gambles on his own ratings value to help the trapped crawlers Li Jun, Zhang, and Li Jun's sister Li Na escape an impossible fight, defying the Maestro, a prince of the porcine Skull Empire, whose grudge against Carl becomes public news. The broadcast reports the crawler population falling below one million. Clearing the local kobold territory, the pair defeat the boss guarding the stairwell, and in its reward room Donut claims Mongo, a juvenile dinosaur she tames, after several bites, into her devoted pet. With Mongo in tow, Carl and Donut take the stairs down to the third floor, where every crawler must soon choose a new race and class.
How does it end? (ending)
The book ends with Carl and Princess Donut completing the second floor and descending the stairs to the third. Behind them, the second floor's great crisis is resolved: Carl destroyed the rampaging Rage Elemental with a vehicle rigged full of bombs and an exploit the whole universe watched, after the creature killed Yolanda and other members of the Meadow Lark group. The surviving Meadow Lark residents, shepherded by Imani and the Andrews brothers, went down ahead of them. On the way to the stairs Carl gambled his celebrity on a rival show to help the trapped crawlers Li Jun, Li Na, and Zhang escape, earning the enmity of the Maestro, a prince of the porcine Skull Empire, and Carl's on-air insolence toward the Empire is already infamous. Donut has tamed Mongo, a young dinosaur who joins the party as her pet. In an epilogue on Odette's talk show, the pair discuss the coming race and class selection that will reshape every crawler on the third floor, and Odette leaves Donut with a private piece of advice about choosing a class. The crawler population has fallen below a million, the game's operators are both profiting from and unsettled by the pair's popularity, and Carl goes down the stairs resolved not just to survive the dungeon but to beat the people who built it.
