Maxwell Kane is a large, withdrawn boy who thinks he has no brain, while his neighbor Kevin Avery is brilliant, physically disabled, and tiny. When Kevin rides on Max's shoulders, they combine Max's strength and Kevin's direction as Freak the Mighty. Their imagined quests help them face bullies, school, and Max's shame about his murderous father, Kenny Kane. After Kenny is paroled, he kidnaps Max and tries to force him to accept a false account of his mother's death. Loretta Lee attempts a rescue, Max finally remembers witnessing the murder, and Kevin saves him by bluffing Kenny with a squirt gun he claims contains acid. Kenny returns to prison. Soon afterward Kevin dies following a seizure; the miraculous medical future he described was an invention. Max withdraws in grief, then uses the blank book Kevin left him to write their adventures, discovering that he can tell their story and move forward.

Maxwell Kane is an unusually large boy who lives with his grandparents, Grim and Gram, and hides from attention in a basement room he calls the Down Under. He has difficulty reading and has accepted other people's judgment that he is unintelligent. His father is in prison, and Max is troubled by how strongly he resembles him. Kevin Avery, a very small boy who uses crutches and leg braces, moves in next door with his mother Gwen. Kevin is brilliant, imaginative, and fascinated by knights, quests, and technology. Max helps him retrieve a mechanical bird from a tree, and Kevin begins teaching him unfamiliar words. Gwen is initially alarmed by Max because she knew his father, but she apologizes and invites him to dinner. The boys discover that Kevin can ride on Max's shoulders and direct him, pairing Kevin's quick mind with Max's strength. Kevin names their combined identity Freak the Mighty.

On the Fourth of July, Max carries Kevin above the crowd so he can see the fireworks. The neighborhood bully Tony D., known as Blade, and his gang threaten them. With Kevin giving directions, Max runs into a muddy millpond where the older boys cannot follow; the police rescue them. Kevin proudly tells the officers that they are Freak the Mighty. The adventure changes how Max sees their friendship and his own abilities. The pair begin treating ordinary trips as quests, with Kevin supplying names and stories while Max provides mobility. When they see Tony D. and his friends put a woman's purse into a storm drain, Kevin decides that recovering it is their next mission. Max is still frightened of trouble and conscious that adults expect the worst from someone his size, but following Kevin gives him a purpose stronger than his wish to remain hidden.

Max lowers Kevin into the storm drain, and together they recover the purse. Its owner, Loretta Lee, lives in the tenements with Iggy Lee, a former motorcycle-gang leader. Returning it brings the boys into a threatening setting where both adults recognize Max as Kenny Kane's son. Loretta tells Kevin about Max's family despite Max's discomfort. At school, bullies use Max's connection to his notorious father against him, but Kevin's presence gives Max the confidence to resist. The school allows the boys to attend classes together because Kevin helps Max concentrate and understand his work. In the cafeteria Kevin has a medical emergency and is taken to the hospital. Afterward he dismisses Max's concern by describing a secret research program that will someday give him a new robotic body. Max believes him, while the reader can see that Kevin uses an elaborate future story to keep fear at a distance.

Grim and Gram learn that Kenny Kane may be released on parole and try to reassure Max while taking extra precautions. On Christmas Eve, after Kevin has given Max a blank book in which to record their adventures, Kenny enters the house and kidnaps his son. He takes Max to Iggy and Loretta's apartment, restrains him, and insists that he was imprisoned unjustly. Kenny presents control as fatherly protection and demands that Max swear obedience. He then moves Max to an abandoned building and ties him where escape is nearly impossible. Max understands that his father means to take him away and that resistance could endanger anyone who helps. Loretta, Iggy, Grim, Gram, Gwen, and Kevin know he is missing, but Kenny's threats and physical power leave Max isolated with the person he has feared throughout his childhood.

Loretta secretly tries to untie Max, but Kenny catches her and begins strangling her. The sight breaks through Max's blocked childhood memory: he saw Kenny kill his mother in the same way. Max tells his father that he remembers, destroying Kenny's claim that the death was an accident and provoking him to attack Max. Kevin arrives with a squirt gun and claims it holds a corrosive chemical. He sprays Kenny in the eyes, allowing Max to escape; the mixture is harmless household material, but Kevin's knowledge and nerve make the bluff work until the police arrive. Loretta survives, and Kenny is arrested. Max's eyewitness memory and the new crimes ensure that his father returns to prison. Freak the Mighty has completed its most dangerous rescue, and Max is finally separated from both Kenny's physical control and the fear that resembling him means becoming like him.

Max and Kevin spend another summer as friends, but Kevin's health continues to fail. On his birthday he suffers a severe seizure and is hospitalized. Max goes to see him expecting the long-promised operation that will place Kevin in a robotic body. Dr. Spivak tells him that the research program was one of Kevin's inventions and that Kevin has died. Overwhelmed, Max runs through the hospital and then shuts himself in the Down Under. He misses the funeral, stays away from school, and avoids his family for months. Grim, Gram, and Gwen give him time, but Max sees no reason to resume life without the friend who supplied its direction. When Loretta encounters him and tells him that doing nothing is pointless, her bluntness reaches him. Max finally opens the blank book Kevin gave him. He writes the story of their friendship and adventures, the account the reader has just finished, and returns to school with a new understanding that his own memory and voice are worth using.

After Kenny Kane is returned to prison, Max and Kevin have a final summer together. Kevin suffers a severe seizure on his birthday and is taken to the hospital. Max expects the experimental body Kevin has often described to save him, but Dr. Spivak explains that no such operation exists: Kevin used the story to imagine a future beyond his worsening condition. Kevin dies, and Max retreats into the Down Under, stops attending school, and avoids nearly everyone. Months later, a chance meeting with Loretta Lee shakes him out of his isolation. Max opens the blank book Kevin gave him and writes down the adventures of Freak the Mighty. By completing their story and returning to school, he begins to grieve without surrendering the friendship that changed him.
