Robert Langdon wakes in a Florence hospital with a head wound and no memory of the past two days, only to be attacked and forced to flee with a young doctor, Sienna Brooks. Haunted by strange visions and armed with a hidden device that projects a modified image of Botticelli's map of Dante's Hell, he follows a trail of clues through Florence, Venice, and finally Istanbul, racing to find what a dead geneticist, Bertrand Zobrist, has left behind. Zobrist, obsessed with overpopulation, has created a biological agent as his radical solution, and Langdon believes he is trying to stop a lethal plague. The truth is stranger: Sienna was Zobrist's former lover and follower with an agenda of her own, and the agent is not a killer but a vector that will randomly render a large portion of humanity infertile. Worst of all, when they reach the hidden site beneath Istanbul, they discover the contagion was released days earlier and has already spread across the world. It cannot be undone, and the story ends with the World Health Organization and Sienna choosing to study and manage this new reality rather than fight a battle already lost.

He is plagued by disturbing, hellish visions. Almost immediately his room comes under attack, and a young doctor, Sienna Brooks, helps him flee for his life. Confused and hunted, and carrying a strange object that will point him toward the imagery of Dante's Inferno, Langdon sets out to piece together what happened to him and to uncover the threat he seems to have been caught up in.

The race to prevent a catastrophe ends with the realization that it has already happened. The agent engineered by Bertrand Zobrist turns out not to be a plague that kills, but a viral vector that spreads through the air and permanently alters human DNA, rendering roughly a third of the population infertile at random, Zobrist's drastic answer to overpopulation. Langdon's missing memory is explained: he had been manipulated, drugged, and steered through a staged scenario by the private organization known as the Consortium and by the head of the World Health Organization, Elizabeth Sinskey, in a desperate effort to locate the threat. Sienna Brooks is revealed to have been Zobrist's devoted partner, initially working to protect his creation, though she comes to fear its uncontrolled release. When the group finally reaches the submerged chamber beneath Istanbul, they find the container already burst; the vector was set loose days before and is now dispersed worldwide, beyond any hope of containment. Faced with an irreversible fact, Sinskey, Langdon, and a chastened Sienna resolve to work together to understand and steward the change rather than pretend it can be reversed. The novel closes on the sobering acceptance of a permanently altered world.
