Years later, the disgraced Liz and criminal associates kidnap Jamie and force him to find a dead man's hidden drug cache. Facing execution, Jamie summons the Therriault entity, which kills his captors and enables his escape. Tia then reveals that her brother Harry is Jamie's biological father. Because Harry has severe early-onset dementia, Jamie is left fearing both the circumstances of his conception and the disease he may have inherited.

Jamie Conklin describes a childhood in which recently dead people remain visible to him for a short time. Because his notes do not contain the ending, Tia faces both professional failure and financial disaster. She takes Jamie to see Thomas's newly dead spirit and asks her son to question him. Bound to answer, Thomas explains the unwritten conclusion. Tia records the information and reconstructs the book well enough to publish it, preserving her business. The episode shows Jamie that even his mother will use the secret when the stakes are high. Liz Dutton, Tia's police-detective partner, also learns enough about the ability to recognize its value.

A bomber named Kenneth Therriault carries out a series of attacks and then dies by suicide. Police fear that another bomb remains hidden and that civilians will die before it can be found. Liz takes Jamie to the dead terrorist and compels him to ask for the device's location. Therriault must tell the truth, and the information allows the bomb to be recovered. Unlike every ordinary dead person Jamie has seen, however, Therriault does not begin to fade. His spirit carries or conceals something inhuman, and the encounter that saved strangers creates a continuing threat directed specifically at Jamie.

Therriault's dead form continues appearing after the normal period has passed. The presence taunts Jamie and proves capable of affecting him in ways the other dead cannot. Jamie understands that the bomber has become a mask for a more ancient predatory entity. Ordinary refusal and distance do not stop it. Seeking an explanation outside the rules he already knows, Jamie learns of a ritual confrontation in which fear, will, and direct mental struggle matter more than conventional weapons. He prepares to face the entity because continued flight will leave it free to choose the time of attack.

Jamie confronts the being associated with Therriault through the ritual he has learned. The struggle forces him into intimate contact with the creature's hunger and with his own fear, but he resists its effort to dominate him. Instead of merely driving it away, he establishes a hold over it and commands it to withdraw. The immediate haunting ends. Jamie returns to ordinary adolescence with the knowledge that the power may still answer him if called. Meanwhile Liz's relationship with Tia has ended, and Liz's police career and drug involvement pull her farther outside the life they once shared.

A disgraced Liz returns with criminal associates and abducts Jamie. She needs his ability to question a recently dead man and locate a hidden cache of drugs. Jamie complies because he has no safe opportunity to escape, and the dead man is compelled to reveal the hiding place. Once Liz has what she wants, Jamie realizes that his knowledge and the risk of identification make him a liability she is prepared to kill. With ordinary rescue unlikely to arrive in time, he deliberately calls on the bound presence linked to Kenneth Therriault. The choice means using the same supernatural predator that once hunted him, with no guarantee that it will stop at the limits he intends.

The Therriault entity turns on Liz and the other kidnappers, killing them and allowing Jamie to escape. Jamie survives because he can command the force that once stalked him, but the experience leaves him aware that control and safety are not the same thing. Afterward Tia reveals the family truth she had concealed throughout his childhood: her brother Harry is Jamie's biological father. Jamie was conceived through incest, and Harry now lives with severe early-onset dementia. The disclosure changes Jamie's understanding of his absent father and gives him a new fear that cannot be questioned away through the dead. He does not know whether he inherited Harry's illness. Looking back, he remains alive and able to tell the story, but the answer about his own mind belongs to a future that has not yet arrived.

After surviving Kenneth Therriault's persistent dead presence, Jamie learns enough to confront and bind the entity rather than letting it control him. That victory becomes necessary when Liz Dutton, disgraced and involved with criminals, abducts Jamie and forces him to question a dead man about hidden drugs. Once the cache is found, Jamie understands that Liz and her associates cannot safely release him. He summons the supernatural force connected to Therriault, and it attacks the kidnappers, allowing Jamie to survive while Liz and the others are killed. The rescue does not make Jamie's gift harmless; it proves that he can use something terrible without being certain he fully controls the cost. Later, Tia finally tells him the central secret in their family: her brother Harry, not an unnamed absent man, is Jamie's biological father. Jamie must absorb that he was conceived through incest and that Harry's early-onset dementia may be hereditary. The novel closes without answering whether Jamie will develop the illness. His fear of what may come gives the retrospective title its final meaning: the most consequential truth is one he can only learn and live with later.
