Five people are killed in a plaza shooting, and the physical evidence points overwhelmingly to former Army infantryman James Barr. Barr denies the charge and insists that Jack Reacher be found, but an attack by other prisoners leaves him comatose. His sister Rosemary rejects the assumption of guilt and hires Helen Rodin, daughter of the district attorney prosecuting the case. Reacher sees the news in Miami and travels to Indiana. He initially tells Helen's team that he has come to help condemn Barr, not clear him. Reacher explains that Barr previously confessed to killing four American soldiers in Kuwait, a case the Army concealed because the victims had themselves committed serious crimes. Barr's denial of the new shooting therefore conflicts sharply with his earlier confession. Grigor Linsky begins watching Reacher and Helen. Although the official evidence still appears decisive, Linsky's employer, Zek, orders an indirect attempt to frighten Reacher away. Sandy helps arrange a bar confrontation, but Reacher defeats the attackers and recognizes that the assault was aimed specifically at him. The need to protect an apparently certain prosecution gives him reason to investigate further.

Reacher finds that the parking garage was a poor firing position and that the evidence was left in an unusually conspicuous trail. He first suspects that Barr was forced to shoot and accept blame, with Rosemary as the likely leverage. A missing piece of video then establishes that someone other than Barr placed a traffic cone used in preparing the garage. Zek's organization recognizes the danger and decides to frame Reacher. Chenko and Vladimir lure Sandy away, kill her, and leave her body at Reacher's former hotel. Emerson treats Reacher as the likely killer, while Donna Bianca helps watch for him. Meanwhile, Helen's questions uncover a gun associate of Barr's named Chenko. The official case against Barr remains strong, but the unexplained cone now provides a concrete path toward another participant. Reacher knows Sandy has been killed and that police are closing in, while Barr's connection to Chenko links the accused man to the same organization monitoring the defense.

At a Kentucky shooting range, Reacher studies retained targets and determines that Barr had disguised worsening performance while Chenko concealed expert ability. Barr's emerging Parkinson's disease convinces Reacher that he could not have made the plaza shots. Reacher reconstructs an impersonation scheme in which a shorter shooter used Barr's van, rifle, clothing, and identifying traces. The firing order suggests that DMV supervisor Oline Archer was the intended victim. Her husband Ted had disappeared after his quarry lost city contracts to an apparently corrupt competitor, and Ted's hidden truck connects his disappearance to Jeb Oliver's barn. Franklin traces the contracts to Specialized Services of Indiana and helps locate Zek's remote house beside its stone plant. Before the team can act, Chenko abducts Rosemary to force testimony against Barr. Ann Yanni supplies Reacher with a false alibi when police arrest him for Sandy's death. Reacher, Helen, Ann, Franklin, and the retired range owner organize a rescue. Helen creates thermal decoys while Reacher enters the house, kills Sokolov, Vladimir, and Grigor Linsky, and throws Chenko from an upper window. He frees Rosemary alive. Chenko's survival is unknown, and Zek remains inside as Reacher returns to confront him. Barr's legal outcome and the wider corruption case remain unresolved.

Reacher's group attacks Zek's isolated house and rescues Rosemary Barr alive. Helen Rodin survives the diversion, while Sokolov, Vladimir, and Grigor Linsky are killed. Chenko is thrown from a third-floor window, but his condition is not confirmed. Zek remains inside, and Reacher goes back to confront him. James Barr appears unable to have fired the plaza shots and therefore appears to have been framed. Reacher links Oline Archer's death and Ted Archer's disappearance to a corrupt city-contract operation, but that theory is not yet independently proved. Barr's legal fate, the official handling of Sandy's murder, the possible leak from Emerson or Alex Rodin, and the ultimate outcomes of Zek and Chenko all remain open at the close.
