John Rambo is living at a Thai monastery when Colonel Trautman asks him to join a mission aiding Afghan resistance fighters against Soviet forces. Rambo refuses to return to war, but changes his mind after Trautman goes alone and is captured by Colonel Zaysen. Guided from Pakistan by Mousa Ghani, Rambo meets Masoud's mujahideen and the boy Hamid. A Soviet attack on their village shows the danger civilians face. Rambo infiltrates Zaysen's fortress, eventually frees Trautman and other prisoners, and fights through Soviet troops during their escape. After killing Zaysen's enforcer Kourov, Rambo and Trautman appear trapped by a larger Soviet force, but Masoud's fighters arrive. In the final battle Rambo uses a tank against Zaysen's helicopter, killing the commander and surviving the collision. Rambo and Trautman leave Afghanistan together after saying farewell to their Afghan allies.

Previously, Vietnam veteran John Rambo survived a violent confrontation after returning to an America in which he could not find a place. His former commander, Colonel Samuel Trautman, persuaded him to surrender and remained his closest ally. Later, Rambo accepted a covert mission to locate American prisoners in Vietnam. Mission director Murdock abandoned him when he found living captives, but Rambo escaped Soviet and Vietnamese forces, freed the prisoners, and brought them to the American base. He then walked away from military authority once more, still deeply loyal to Trautman but unwilling to trust the institutions that had used them both.

John Rambo has settled at a Buddhist monastery in Thailand, supporting the monks through construction work and earning extra money in dangerous stick-fighting contests. Colonel Trautman finds him there and asks him to join an operation in Afghanistan, where Soviet forces are waging war against resistance fighters and civilians. Rambo refuses. He respects Trautman but believes he has finally found a way to live without returning to combat. Trautman proceeds without him. American official Griggs later brings news that the mission failed and Trautman has been captured. The government considers a direct rescue too dangerous. Loyalty overcomes Rambo's wish for peace: he takes the mission information and decides to enter Afghanistan himself.

Trautman is held in a Soviet fortress commanded by Colonel Zaysen. Zaysen interrogates him about American support for the Afghan resistance, but Trautman refuses to cooperate and warns that Rambo will come for him. Rambo crosses into Pakistan, obtains weapons, and meets Mousa Ghani, an Afghan guide with the contacts needed to move through the border region. Mousa explains that a rescue cannot be separated from the wider war: Soviet patrols control the roads, villages have been destroyed, and local fighters have their own urgent responsibilities. Despite the risks, Mousa agrees to lead Rambo toward the resistance community nearest Zaysen's base.

Rambo and Mousa reach fighters led by Masoud and meet Hamid, a boy eager to take part in the resistance. Masoud is willing to discuss Trautman's prison but cannot sacrifice all his people for one foreign captive. The reality of the occupation becomes immediate when Soviet helicopters attack the community. Rambo joins the defense and helps civilians escape, but the assault inflicts casualties and leaves the survivors with still fewer resources. Masoud's fighters must protect their families and continue their own struggle. Rambo prepares to attempt the rescue with only limited help, while Hamid's determination to accompany him creates another life for which he must take responsibility.

Rambo, Mousa, and Hamid approach the Soviet fortress through the surrounding caves and defenses. Their first attempt reaches prisoners but triggers an alarm before they can complete the rescue. Soviet troops respond in force, and the group must withdraw under fire. Hamid is wounded, reinforcing that the boy cannot safely continue. Rambo sends him and Mousa away rather than expose them to another assault. He remains behind alone, studies the base, and prepares to re-enter it without expecting outside support. Inside, Zaysen continues trying to break Trautman, unaware that Rambo has chosen the direct and nearly impossible approach Trautman predicted.

Rambo penetrates the fortress, frees Trautman, and releases other captives while Soviet troops converge on them. The rescue becomes a running battle through the base and into the cave system outside. Rambo and Trautman fight as a team again, using captured weapons and the terrain to offset the larger force. Zaysen commits men and aircraft to stop their escape. His enforcer Kourov corners Rambo in the caverns, where the two fight at close quarters until Rambo kills him. Although the fugitives break out of the caves, Zaysen's remaining troops and armored vehicles surround them on open ground, leaving Rambo and Trautman facing an army with little cover and no apparent route away.

Masoud's mujahideen charge into the battlefield on horseback, giving Rambo and Trautman the support they seemed to have lost. The Afghan fighters engage the Soviet troops while Rambo takes control of a tank. Zaysen attacks from his helicopter, and Rambo drives directly toward him. Tank and aircraft collide; Zaysen is killed, while Rambo emerges alive from the wreckage. With the Soviet force defeated, the surviving resistance fighters regroup. Rambo and Trautman thank Mousa and Masoud and say farewell to Hamid, whom Rambo leaves with a small token of friendship. The two Americans depart Afghanistan together. Trautman has been rescued and their bond reaffirmed, but Rambo's future remains outside ordinary military or civilian structures.

Rambo and Trautman escape Zaysen's fortress with other prisoners but are pursued across the Afghan landscape. Rambo kills Kourov during close combat in a cavern, and the fugitives reach open ground only to face Zaysen's massed troops, armor, and helicopter. Masoud and the mujahideen arrive on horseback and turn the confrontation into a battle. Rambo takes control of a tank and drives directly against Zaysen's helicopter; the collision destroys both vehicles, but Rambo survives and Zaysen is killed. With the Soviet force defeated, Rambo and Trautman say farewell to Mousa, Masoud, Hamid, and the fighters. Rambo gives Hamid a token of friendship before leaving Afghanistan with Trautman. The rescue restores the bond between the two soldiers, but Rambo again departs without returning to a conventional American life.
