John Rambo is released from prison for a covert mission to photograph a Vietnamese camp where American prisoners may remain. Mission chief Murdock orders him to gather evidence only, while Colonel Trautman watches over the operation. Guided by Vietnamese agent Co Bao, Rambo infiltrates the camp and rescues a prisoner named Banks. Murdock, fearing the political consequences of proof that captives were abandoned, orders the extraction helicopter to leave them behind. Rambo is recaptured and tortured by Soviet officer Podovsky, but Co frees him. After she is killed, Rambo destroys the forces hunting him, returns to the camp, and rescues the remaining prisoners. He defeats Yushin in an aerial battle and flies the captives to the American base. There he confronts Murdock and demands action for the men still missing, then leaves rather than returning home.

Previously, Vietnam veteran John Rambo returned to the United States unable to find a place in civilian life. A confrontation with a small-town sheriff escalated into a manhunt in the mountains, where the survival and combat training Rambo learned as a Green Beret made him extraordinarily dangerous. His former commander, Colonel Samuel Trautman, tried to prevent further deaths and finally persuaded him to surrender. Rambo survived the confrontation but was imprisoned, while Trautman remained his closest link to the military past that shaped him.

John Rambo is performing hard labor in prison when Colonel Trautman visits with an offer. Intelligence suggests that American prisoners may still be held in Vietnam, and the government wants a veteran capable of entering the country without attracting attention. If Rambo accepts and succeeds, he can earn his freedom. He agrees, partly because the missing men represent comrades his country may have left behind. At the operational base in Thailand, mission director Marshall Murdock explains that Rambo is to reach a former prison camp, photograph any prisoners he sees, and leave without engaging the guards. Murdock presents the mission as tightly controlled reconnaissance. Rambo distrusts the officials but accepts the limits and prepares to return to the landscape of his war.

Rambo's insertion is disrupted when equipment catches as he jumps from the aircraft, forcing him to abandon much of the gear selected for the mission. He continues with weapons and skills he knows he can rely on. Inside Vietnam he meets Co Bao, the local operative assigned to guide him. She moves him through river traffic and hostile territory while helping him avoid patrols. Co is capable and independent, and she and Rambo develop trust despite the speed and danger of the mission. As they approach the camp, Rambo learns that it is not the empty ruin the planners expected. Guards remain active, making clear that the intelligence and Murdock's assurances did not describe the whole situation.

Rambo infiltrates the prison camp and finds living American captives in severe condition. Rather than leave with photographs, he frees one prisoner, Banks, and begins carrying him toward extraction. The choice violates Murdock's explicit order but answers the real purpose for which Rambo believed he had been sent. Vietnamese troops pursue them as Co helps them reach the rendezvous. An American helicopter approaches, and rescue appears possible. When Murdock learns that Rambo has brought a live prisoner, however, he orders the flight crew to break off. Proof of surviving prisoners would expose the falseness of the mission's assumptions and create consequences Murdock is unwilling to face. Rambo and Banks are abandoned on the ground and fall back into enemy hands.

Rambo is returned to the camp, where Soviet officer Podovsky takes charge of his interrogation. Podovsky and the powerful soldier Yushin torture him and attempt to force him to radio the American base with a warning to stop further operations. Rambo reaches Murdock by radio but turns the demanded message into a personal threat against the official who betrayed him. Co Bao infiltrates the camp in disguise and frees Rambo. During their escape, the two acknowledge their attachment and imagine a future beyond the mission. That possibility ends almost immediately when enemy fire kills Co. Rambo buries her, takes her pendant, and abandons any remaining attempt to withdraw quietly. Grief and betrayal turn his escape into a campaign against the soldiers hunting him.

Using the forest, improvised traps, and captured weapons, Rambo eliminates the patrols pursuing him. He works his way back toward the prison camp instead of escaping alone. There he attacks the guards and releases the surviving American prisoners. Rambo and the captives seize a helicopter and head toward the Thai base, but Yushin pursues them in a heavily armed Soviet gunship. The fight appears to force Rambo's damaged helicopter down. He uses the apparent crash to draw Yushin close, returns fire, and destroys the pursuing aircraft. The former prisoners continue toward the American command with direct proof of everything Murdock tried to suppress.

Rambo lands at Murdock's base with the rescued prisoners, exposing the operation's deception. He storms into the command center, destroys its equipment, and confronts Murdock with a knife. Rather than kill him, Rambo orders him to locate and recover the other Americans still missing in Vietnam. The freed men receive care, and Trautman asks Rambo what he will do now that he has earned his release. Rambo rejects an easy return to the country that sent soldiers to war and then tried to forget them. He wants the nation to value its veterans as much as they valued it, but he does not believe that wish has been fulfilled. Leaving Trautman and the base behind, he walks away alone, free but still without a settled home.

After Co Bao is killed, Rambo wages a personal campaign against the Vietnamese and Soviet forces pursuing him. He reaches the prison camp, frees the American captives, and takes a helicopter for the flight back to Thailand. Yushin attacks in another gunship, but Rambo survives the aerial battle and destroys his pursuer. He lands at Murdock's base with the prisoners whom the operation was designed not to find. Rambo then wrecks the command center and forces Murdock at knifepoint to acknowledge the men still missing in Vietnam and to pursue their rescue. He does not take the offered path back into ordinary American life. The surviving prisoners are safe, Trautman remains the person who best understands him, and Rambo walks away from the base still alienated from the country for which he fought.
