Cambridge scholar Horace Holly raises Leo Vincey and, when Leo turns twenty-five, opens a family relic describing an ancient ancestor murdered by a queen in Africa. Holly, Leo, and their servant Job follow the clue, survive a wreck, and travel inland with Mahomed. Among the Amahagger, Ustane claims Leo as her husband, while violence at a feast kills Mahomed. The survivors are taken to ruined Kôr, ruled by the veiled Ayesha, or She-who-must-be-obeyed. Ayesha says she gained immortality two thousand years earlier and killed Kallikrates when he rejected her; she believes Leo is his returned form. She kills Ustane, heals Leo, and wins both men's devotion through her supernatural beauty. At the life-giving fire, Ayesha enters first to reassure Leo, but a second exposure makes her age and die, promising to return. Job dies of fright, and Holly and Leo escape without resolving whether Ayesha's promise is true.

Horace Holly, a physically powerful and scholarly Cambridge man, receives a final request from his dying friend Vincey: he must raise Vincey's five-year-old son Leo and give him a sealed family chest on his twenty-fifth birthday. Holly accepts and brings up the remarkably handsome boy with the help of his servant Job. Twenty years later, Leo opens the chest. Its objects include an ancient inscription associated with Amenartas, an Egyptian princess, and Kallikrates, a priest of Isis and Leo's remote ancestor. The record says that Kallikrates fled with Amenartas to Africa, where a mysterious queen desired him and killed him when he remained loyal to his wife. Amenartas charged their descendants to travel to the place and avenge him. A resemblance between Leo and the ancestral image strengthens the mystery. Holly, Leo, and Job decide to follow the geographical clues rather than leave the inherited task unanswered.

The three Englishmen reach the East African coast and engage Mahomed as sailor and interpreter. Their voyage toward the location described in the relic ends in disaster, but they survive and reach a remote shore. Moving inland through dangerous marsh country, they encounter the Amahagger, a people who live in caves and acknowledge a distant female ruler. The elderly Billali exercises authority among them and receives the strangers with cautious hospitality. Ustane, a young Amahagger woman, chooses Leo as her husband according to local custom, and Leo accepts her affection. The travelers learn that the ruler known as She-who-must-be-obeyed has already heard of their arrival and expects them to be brought before her. Holly's party is therefore drawn deeper into a society whose customs they scarcely understand, guided by people who show far more fear of their queen than of any ordinary chief.

Hospitality among the Amahagger turns violent during a communal feast. A struggle breaks out around a deadly local practice, and Mahomed is killed. Holly and Leo defend themselves with firearms until Billali's return restores order and saves the remaining visitors. Leo is badly wounded and becomes dangerously ill, while Ustane refuses to leave him. Billali leads the party onward toward the ancient ruined city of Kôr, where his queen lives among tombs and caverns left by a vanished civilization. Holly is summoned alone into the presence of Ayesha, the veiled woman obeyed throughout the region. She demonstrates uncanny knowledge, speaks of a life extending across many centuries, and takes particular interest in the strangers' arrival. Her command combines political rule with powers Holly cannot explain, and even before seeing her face he recognizes that the expedition has reached the source of the Vincey legend.

Ayesha reveals more of her history to Holly. She says she has lived for roughly two thousand years after entering a force that renews life, and that she has waited in Kôr for the return of Kallikrates, the man she loved and killed in jealousy. She believes Leo's resemblance and lineage show that he is Kallikrates restored to her. When she unveils, her beauty overwhelms Holly's judgment despite his loyalty to Leo. Ayesha visits the sick young man and uses her knowledge to restore him. Ustane, however, insists that Leo is her husband and will not obey Ayesha's order to leave him. The queen kills Ustane through her supernatural power. Leo is initially horrified, but after Ayesha reveals herself to him he too falls under her influence. The travelers now understand that the ancestral account was substantially true, but the promised act of vengeance has been displaced by Leo's bond with the woman it named as murderer.

Ayesha shows Holly and Leo the remains and traditions of Kôr, presenting its ruined greatness as one episode in the long span of history she has witnessed. She intends Leo to share her extended life and rule with her. Holly, though aware of her cruelty and of Ustane's death, remains unable to free himself emotionally from the effect of Ayesha's presence. Leo likewise accepts the journey toward the source of her immortality. Job follows in terror and distrust, loyal to his companions but convinced that the queen's powers are dangerous. Ayesha leads the party through difficult underground passages toward a hidden cavern where a moving pillar of fire appears at rare intervals. She explains that Leo must enter the force so that he can become enduringly young as she is. The expedition that began as a mission of inherited revenge has therefore become a search for immortality under Ayesha's direction.

The party reaches the cavern of the life-giving fire. Leo hesitates to enter the mysterious force, so Ayesha steps into it first, expecting to renew and strengthen the immortality it granted her long ago. The result is the reverse: her youth vanishes, her body rapidly contracts into extreme age, and she dies after promising Leo that she will come again. The sight overwhelms Job, who dies from shock. Leo and Holly are left alive but without the immortality they had been offered, and with no certainty about the meaning of Ayesha's final promise. They eventually escape the caverns and the ruined city, making their way back through the difficult country they crossed on arrival. Job is left buried in Kôr. Holly records the adventure after their return, unable to dismiss either Ayesha's demonstrated powers or the possibility that death may not end her long connection to Kallikrates and Leo.

Ayesha leads Leo, Holly, and Job through the ruins of Kôr to the cavern where a mysterious life-giving fire periodically appears. Because Leo hesitates to enter it, Ayesha steps into the flame to demonstrate that it is safe and to renew her immortality. Instead, the second exposure reverses the effect of the first: before the men she rapidly loses her youth, shrinks into extreme age, and dies. She tells Leo that she will return before the change is complete. Job dies from the shock and terror of witnessing her transformation. Holly and Leo eventually make their way back through the ruins and marshes, carrying the memory of Ayesha and leaving Job buried in Kôr. Leo survives without attaining immortality, and both men remain marked by their encounter. Holly closes his account uncertain whether Ayesha was destroyed or merely passed into another state, while her promise to return leaves Leo's connection to her unresolved.
