Tally is now a Special, one of the Cutters that Shay leads for Dr. Cable, keeping her mind cold and clear through pain. When Zane, still a pretty and damaged by the half-cure he took, plans to escape the city with the Crims, Special Circumstances lets it happen so that he can be followed to the New Smoke. The plan collapses, and Tally ends up running with Zane to Diego, a city that has taken the cure and lets people look and think as they please. There Zane's injured brain fails and he dies, and Tally's own city attacks Diego outright, bombing a place that had forgotten what war was. Tally turns on her makers, hunts down Dr. Cable and forces the cure on her rather than killing her. The operations end, the cure spreads, and the cities wake up. Rather than join the new order, Tally and David keep their Special bodies and go into the wild as its self-appointed guardians, warning the freed world not to repeat what the Rusties did.

Previously: in a city that gives everyone beauty surgery at sixteen, Tally Youngblood was blackmailed by Special Circumstances into betraying the Smoke, a settlement of people who refused the operation, then chose the runaways' side after learning that the surgery also leaves lesions in the brain that make people docile. The settlement was destroyed and David's father killed. Made pretty herself, Tally later found the two-pill cure she had hidden for her future self and split it with Zane, which cleared both their minds and injured his. They led an escape from New Pretty Town that scattered; Tally was stranded in the wild, found a village the cities keep deliberately primitive, and went back for Zane. Special Circumstances caught them both. Shay, already rebuilt as a Special, asked for Tally to be given the same operation, and Tally ended that book remade: strong, fast, cold, and working for the organization she had fought.

Tally is a Cutter, one of Shay's handpicked Specials, living in a state of contemptuous clarity that she maintains by cutting herself whenever her thoughts blur. The Cutters patrol the city for anything the surgery cannot smooth away. Shay has learned that Zane, still a pretty and now leading the Crims, is planning to escape and reach the New Smoke, and she persuades Dr. Cable to let it happen so that the runaways can be followed to their base. Tally is uneasy: her Special mind despises Zane's weakness, and something older in her does not.

The Crims prepare their escape, breaking into a city armoury for equipment, and Tally moves among them in secret to make sure they succeed. Zane is visibly failing, shaking and exhausted from the half-cure he swallowed, but he refuses to be left behind. When the Crims finally get out of the city and into the wild, the Cutters shadow them, waiting to be led to the New Smoke.

The plan comes apart. The Crims are intercepted, and Tally, whose loyalties are pulling in two directions at once, ends up running with Zane instead of hunting him. The pair make for Diego, a distant city where the cure has spread and the rules Tally grew up with no longer apply. What they find is a place where people choose their own faces and bodies, argue openly about how they should be governed, and treat runaways as citizens rather than criminals.

In Diego, Tally is treated as both a curiosity and a danger while its doctors examine what Special Circumstances did to her body and her mind. She meets the New Smoke's people again, David among them. Zane is admitted for care, and the harm the half-cure did to him proves worse than anyone had admitted. Meanwhile Tally's own city has noticed how many of its runaways end up in Diego, and Dr. Cable has no intention of tolerating a neighbour that undoes her work.

Zane's injured brain fails and he dies. Tally, who blames herself for splitting the pills and Dr. Cable for everything else, has nothing left holding her back. At about the same time her city moves against Diego: Special Circumstances launches an assault, with Specials and firebombing, that turns a city which had barely any idea what war looked like into a disaster. Maddy and the others work through the casualties, and Tally chooses her side for good.

With Diego burning and every free city alarmed, Tally turns on the organization that made her. She goes back to her own city after Dr. Cable, and Shay's loyalties are forced into the open as well. The cure Maddy created has by now become something that can be spread widely rather than swallowed one dose at a time, and Tally's plan is not to kill Dr. Cable but to change her, taking away the mind that ordered the attack.

Tally reaches Dr. Cable and forces the cure on her, ending Special Circumstances as the thing it was, and the cure spreads through the city and past its borders. The operations stop, and people wake up able to think for themselves for the first time in generations, with no idea yet what to do about it. Shay takes a place in the new order. Tally does not: she and David keep their Special bodies and abilities on purpose and go out into the wild, appointing themselves its guardians and putting the newly free world on notice that if it starts consuming the planet the way the Rusties did, they will be there to stop it.

The attack on Diego pushes Tally into open revolt against the organization that built her. She goes back to her own city after Dr. Cable, and her answer is not to kill her but to force the cure on her, taking away the mind that gave the orders. The cure spreads, the operations stop, and city after city is left with a population able to think clearly and no institution designed to cope with that. Zane is dead, killed by the damage the half-cure did to his brain. Maddy and the people of the New Smoke are absorbed into the new arrangements, and Shay takes her place inside the changed system. Tally refuses it. She keeps her Special body and her Special senses deliberately, and she and David walk out into the wild together as its self-appointed guardians, leaving behind a plain warning that they will be watching and that the freed cities will not be allowed to strip the world the way the Rusties did. The trilogy closes with the old order gone, the future undecided, and Tally standing outside both by choice.
