Salander survives the head wound that ended the previous book and recovers in a hospital where her father, Zalachenko, is being treated one floor away. The state, driven by the secret security-police cell that has protected Zalachenko for decades, plans to have her convicted and locked away as insane. Blomkvist mounts a counterattack, gathering proof that Salander was illegally institutionalized as a child to bury the truth about her father, and enlists his sister Annika Giannini as her defense lawyer while a constitutional-protection officer, Monica Figuerola, investigates the rogue cell from inside the security police. An old operative murders Zalachenko in his hospital bed and then kills himself. At trial, Annika dismantles the psychiatrist Peter Teleborian, exposing his falsified reports and his own crimes, and the conspiracy collapses: its members are arrested and Salander is acquitted and finally declared legally competent, free of guardianship for the first time. Niedermann, the killer half-brother still at large, is hunted down; Salander traps him and he is killed by the criminals he had betrayed. She and Blomkvist reconcile as friends.

Previously in the trilogy: journalist Mikael Blomkvist and hacker Lisbeth Salander first joined forces to solve a decades-old family murder mystery, after which Salander exposed a corrupt financier and secretly stole a fortune from him, clearing Blomkvist's name. In the second book, Salander was framed for three murders, including the killing of her abusive former guardian and of two journalists preparing a sex-trafficking expose for Millennium. Investigating, she and Blomkvist uncovered that the trafficking boss known as Zala was Alexander Zalachenko, a defected Soviet intelligence officer secretly protected for decades by a hidden cell within the Swedish security police. The revelation was intensely personal: Zalachenko is Salander's father, and the murders had been carried out by Ronald Niedermann, a giant blond killer immune to pain who is her half-brother. The same officials had once had the young Salander committed to a psychiatric institution to conceal the truth. In a final confrontation at Zalachenko's farm, Salander was shot in the head and buried alive, dug herself out, and struck her father down with an axe before Niedermann fled and Blomkvist found her near death. This book opens with both Salander and Zalachenko hospitalized and Niedermann on the run.

Salander recovers from her wounds while confined and awaiting trial. The secret security-police faction, desperate to keep its decades-old protection of Zalachenko hidden, works to have her convicted and committed. To silence Zalachenko himself, an aging former member of the cell shoots him dead in his hospital room and then takes his own life. Blomkvist, aided by his sister Annika Giannini as defense counsel and by the security-police investigator Monica Figuerola, assembles overwhelming evidence: that Salander was sane all along and had been wrongfully institutionalized as a girl to bury the truth about her father, and that the psychiatrist Peter Teleborian had falsified her records. At the climactic trial Annika exposes Teleborian's forgeries and his possession of child pornography, discrediting him completely; he is arrested, and the members of the illegal cell are exposed and taken into custody. Salander is acquitted of all charges and, for the first time, declared legally competent and freed from state guardianship. The fugitive Niedermann is finally cornered when Salander tracks him to a warehouse and immobilizes him, and he is killed there by the criminal bikers he had earlier betrayed. Salander regains control of her life and her stolen fortune, and she and Blomkvist restore their friendship on new, more equal terms.
