Psychotherapist Theo Faber joins the Grove psychiatric unit to treat Alicia Berenson, a painter who stopped speaking after shooting her husband Gabriel. He lowers her medication, studies her diary, and investigates relatives and friends while his own marriage deteriorates after he discovers his wife Kathy's affair. Alicia eventually speaks and describes a masked intruder who tied up the couple and forced Gabriel to choose which of them should die; Gabriel chose himself, and Alicia then killed him. Theo is the intruder. He had followed Kathy's lover, Gabriel, and staged the confrontation as revenge, unknowingly recreating Alicia's deepest fear of being sacrificed. When Alicia recognizes him, Theo gives her a morphine overdose that leaves her comatose. He believes he has concealed the crime, but Alicia's diary names him. Inspector Allen brings Theo the recovered account, and Theo realizes that his arrest is imminent.

Painter Alicia Berenson is found beside her husband Gabriel, a fashion photographer who has been tied to a chair and shot five times. Alicia has cut her wrists and never speaks again, including at her trial. Her silence and a self-portrait titled for the mythic Alcestis turn the case into a public obsession. Six years later, forensic psychotherapist Theo Faber takes a job at the Grove, the secure unit holding her, specifically hoping to treat her. Theo understands therapy through his own history of an abusive father, suicidal despair, and the treatment that helped him survive. At the Grove he meets clinical director Professor Diomedes and a divided staff, some of whom regard Alicia as unreachable.

Theo persuades the Grove's clinicians to reduce Alicia's heavy medication so that meaningful contact may become possible. She remains silent in their sessions, sometimes using drawing or physical action instead of speech, and her volatility makes the work risky. Theo begins investigating outside the consulting room despite warnings about boundaries. He speaks with Gabriel's brother Max, who represented Alicia, and with people connected to her art. Alicia's recovered diary supplies another perspective on the weeks before the murder: she loved Gabriel but became convinced that an unknown man was watching her home. Gabriel treated the fear as a symptom rather than a real threat, leaving Alicia increasingly isolated.

Theo continues reconstructing Alicia's relationships. Gallery manager Jean-Felix resented Gabriel's influence and believed Alicia should remain devoted to her work. Max's attachment to Alicia also complicates the family account, while the Grove staff carry conflicts of their own. Theo travels to Alicia's childhood home and meets her cousin Paul and aunt Lydia. He learns that Alicia's mother died after driving a car into a wall with the young Alicia inside. Later, in grief, Alicia's father openly wished that his daughter had died instead. Theo sees this as a foundational wound: Alicia came to expect that a person who claimed to love her might choose her death. Meanwhile, Theo discovers that his wife Kathy is having an affair and secretly follows her, concealing the crisis from colleagues and readers of his professional composure.

Alicia begins to respond more directly to Theo and eventually gives him her diary. Her account of being watched intensifies as the murder approaches, and Theo treats its masked figure as the key to her silence. His own pursuit of Kathy and her unidentified lover becomes increasingly obsessive, mirroring the surveillance described in Alicia's pages. At the Grove, institutional pressure threatens Theo's work, while Alicia's vulnerability and the hostility around her raise the danger. The narrative's two strands—Theo's troubled marriage and Alicia's account of the weeks before Gabriel's death—draw closer even though Theo continues presenting them as separate histories.

Alicia finally speaks. She describes a masked man entering the house, restraining her and Gabriel, and staging a choice in which Gabriel could sacrifice himself to save her. Gabriel instead pleaded for his own life. The intruder left them alive, but the choice confirmed Alicia's deepest conviction that those she loved would abandon her; she shot Gabriel. Theo recognizes more than he admits in responding to this account. When Alicia later makes clear that she recognizes him, she becomes a danger to the identity and career he has constructed. Soon afterward she is found after a morphine overdose and falls into a coma. Theo encourages suspicion of Elif and uses his clinical authority to distance himself from the act.

Theo is the masked intruder. After discovering that Kathy's lover was Gabriel, he followed him to the Berenson home, tied up Gabriel and Alicia, and forced the choice as punishment. He did not kill Gabriel, but his cruelty triggered the trauma that ended with Alicia firing the gun. Years later, Alicia recognized him at the Grove. Theo injected her with morphine to silence her permanently and tried to make the overdose resemble another patient's crime. Alicia had already written the final sequence in her diary, identifying Theo and describing what he did. Chief Inspector Allen recovers the diary and brings it to Theo. As Allen explains that Alicia was able to leave this testimony, Theo realizes that his confident account has become evidence against him and that the police now know the truth.

Years earlier, after discovering that his wife Kathy was having an affair with Gabriel, Theo followed Gabriel, entered the Berensons' house in disguise, tied up the couple, and forced Gabriel to choose between his own death and Alicia's. Gabriel chose to save himself. Theo left without shooting either of them, but Alicia, devastated by Gabriel's betrayal, killed him. When Alicia communicates that she knows Theo's identity, he injects her with enough morphine to leave her in a coma and tries to redirect suspicion toward another patient. Alicia has already recorded the truth in her diary. Chief Inspector Allen obtains it and visits Theo, who understands that the account will expose both the original break-in and his attack at the Grove. Snow falls outside as Theo awaits the consequences, echoing the peaceful memory with which he began his narrative.
