An unnamed narrator addresses an unseen listener, insisting that heightened senses and careful planning prove sanity rather than madness. The narrator says there was no hatred or desire for the old man's money, only an intolerable fixation on his clouded, pale eye. For seven nights the narrator enters the man's room with extreme caution but finds the eye closed. On the eighth night a small noise wakes him. After a long silence, the narrator opens the lantern enough to expose the eye and hears what seems to be a rapidly beating heart. Fearing the sound will alert a neighbor, the narrator kills the old man, dismembers the body, and hides it beneath the floorboards. Three police officers arrive because a cry was reported. The narrator welcomes them, explains the absence of the old man, and seats them over the concealed remains. While they converse, a faint beating grows louder in the narrator's perception. Convinced the officers hear it and are mocking the deception, the narrator collapses under the pressure, confesses, and orders them to uncover the body.

After killing and dismembering the old man, the narrator hides the remains beneath the floorboards and confidently welcomes three police officers into the house. The officers find nothing suspicious and sit in the room directly above the hiding place. During their casual conversation, the narrator begins to hear a faint, regular beating that seems to grow steadily louder. Convinced that it is the dead man's heart and that the officers also hear it but are silently mocking the attempt at concealment, the narrator loses all composure. The sound is not confirmed by anyone else and may be guilt, terror, or an auditory hallucination. Unable to endure it, the narrator confesses and tells the officers to lift the boards, exposing the body and undoing the carefully planned crime.
