SPECTRE, led by Ernst Stavro Blofeld, steals a NATO bomber carrying two nuclear weapons and threatens to destroy major cities unless an enormous ransom is paid. Emilio Largo directs the field operation from the Bahamas, hiding the aircraft and warheads beneath the sea. After an enforced health cure introduces him to one of the conspiracy's agents, James Bond follows a clue to Nassau and investigates Largo with Felix Leiter. Bond befriends Largo's mistress Domino and proves that SPECTRE murdered her brother after using him to steal the bomber. Domino agrees to help but is discovered and tortured. Bond and allied divers attack Largo's underwater force as it moves a bomb into position. Domino escapes and saves Bond by killing Largo, while the coerced scientist Kotze turns against SPECTRE. The weapons are recovered and the ransom plot fails, but Blofeld remains free.

James Bond is an experienced officer of the British Secret Service who has repeatedly survived assignments against organized crime and hostile intelligence services. M remains his demanding chief, and former CIA officer Felix Leiter is a trusted American ally from earlier operations. Bond begins this case between missions, with accumulated strain showing in his medical results rather than with a specific unfinished case from the preceding books.

M is dissatisfied with Bond's health report and orders him to spend two weeks at Shrublands, a clinic built around diet, rest, and physical treatment. Bond resents the regime but takes an interest in his physiotherapist, Patricia Fearing. Another patient, Count Lippe, behaves secretively and has a criminal association that Bond recognizes. Lippe traps Bond in a traction machine and turns the controls high enough to threaten his life. Patricia releases him in time. Bond retaliates by using a treatment on Lippe in a way that leaves the count badly burned, turning their mutual suspicion into a private war.

Ernst Stavro Blofeld convenes SPECTRE and demonstrates the combination of careful accounting and lethal discipline with which he controls the organization. Its new operation is nuclear extortion. Giuseppe Petacchi, an Italian airman with access to a NATO training flight, kills and replaces a crew member, poisons the others, and diverts a bomber carrying two atomic weapons. He lands it in shallow water near the Bahamas, where Emilio Largo's team removes the bombs and hides the aircraft. Largo then kills Petacchi, eliminating a witness. SPECTRE sends the Western governments a demand for a vast payment and threatens to destroy cities if the deadline passes.

Britain and the United States begin a desperate search for the missing aircraft and bombs. Bond studies the available reports and connects Count Lippe's activity at Shrublands with the timing of the theft. His reasoning points toward the Bahamas, and he is sent to Nassau. There he begins watching Largo, a conspicuously wealthy man whose fast yacht, the Disco Volante, could support an operation at sea. Bond also meets Largo's mistress, Dominique Vitali, known as Domino, and learns that the dead airman Petacchi was her brother. The evidence is incomplete, but Largo and his vessel become the center of Bond's inquiry.

Felix Leiter joins Bond in Nassau under civilian cover, and the two investigate by air and water. They search likely stretches of sea for the missing bomber while Bond cultivates his acquaintance with Domino and accepts Largo's hospitality. Bond's underwater reconnaissance around the Disco Volante confirms that Largo's apparently sporting expedition is guarded like a military operation. He survives an attempt to kill him during the dive and becomes convinced that the yacht is connected to the hidden weapons. Bond and Leiter locate signs of the submerged aircraft, bringing the search close to proof even as the ransom deadline approaches.

Bond tells Domino that Largo's organization used and murdered her brother, supporting the accusation with evidence recovered during the search. Grief and anger overcome her remaining loyalty to Largo, and she agrees to carry a signal that can reveal where the Disco Volante takes the bomb. Largo discovers the betrayal, confines her, and tortures her for information. Bond and Leiter meanwhile assemble the naval and diving support needed to intercept SPECTRE underwater. The coerced physicist Ladislav Kotze also begins looking for a way out, while Largo commits his men to moving a warhead toward its target.

Bond joins allied combat swimmers in an underwater attack on Largo's force as it transports one of the nuclear weapons. The fight is confused and brutal, but reinforcements help destroy the SPECTRE team and secure the bomb. Largo gets the better of an exhausted Bond and is about to kill him when Domino, having survived her captivity, shoots Largo with a spear gun. Both bombs are recovered before SPECTRE can carry out its threat, defeating the extortion plan. Blofeld remains outside the field operation and escapes the disaster. Bond and Domino are hospitalized, where they begin recovering from the battle and its personal losses.

Bond joins the underwater assault on Largo's men as they move one of the warheads. The opposing swimmers fight with knives, spears, and explosives, and the arrival of allied support turns the battle against SPECTRE. Largo nearly kills Bond, but Domino, freed after being tortured for helping the investigation, shoots Largo with a spear gun. Both nuclear weapons are recovered before the ransom deadline, ending the immediate threat, although Blofeld is not among the defeated field team and remains at large. Bond and Domino are taken to hospital after their injuries, sharing the exhausted aftermath of an operation that cost her brother and nearly killed them both.
