As an old man in a nursing home, Jacob Jankowski recalls the year he ran away with a circus. In 1931, days before finishing his veterinary studies at Cornell, Jacob is left orphaned and broke when his parents die in a car crash. He jumps a train that turns out to belong to the Benzini Brothers circus, a shabby, cost-cutting show run by the tyrannical Uncle Al, and becomes its veterinarian. He falls in love with Marlena, the show's star equestrienne, who is married to August, the charming but violently unstable animal boss. The circus takes on an elephant named Rosie, and Jacob discovers her hidden intelligence even as August abuses her. Amid brutal working conditions, where troublesome laborers are thrown from the moving train, the love triangle and cruelty build toward disaster. A stampede during a performance brings the show crashing down, August is killed by the elephant, and Jacob and Marlena escape together to build a life. In the present, the aged Jacob makes his own bid for one last adventure.

Framed by the memories of Jacob Jankowski, a man in his nineties living in a nursing home, the story returns to 1931, when Jacob was a young veterinary student at Cornell. On the eve of his final examinations, his parents are killed in a car accident, and he learns that the family is ruined and he is left with nothing. Overwhelmed, he walks out of his exams and his old life and, wandering in the dark, hops aboard a passing train. It proves to be the traveling home of the Benzini Brothers Most Spectacular Show on Earth, a struggling Depression-era circus. Taken on as the show's veterinarian, Jacob is thrown into a strange, hard world of performers, laborers, and animals, ruled by the ruthless owner Uncle Al and the mercurial animal boss August, and lit up by the presence of August's wife, Marlena, the show's beautiful star rider, to whom Jacob is drawn from the first.

The tensions aboard the Benzini Brothers circus erupt in catastrophe. After a purge of workers thrown from the moving train, someone releases the menagerie animals during a show, and a stampede turns the big top into chaos. In the confusion, the elephant Rosie kills the cruel August, freeing Marlena from her abusive husband. Uncle Al's mismanaged show collapses in the wake of the disaster. Jacob and Marlena leave together with Rosie and several of the animals, and Jacob at last completes his veterinary training. They marry, raise a large family, and Jacob works for years as a zoo veterinarian, keeping the circus animals cared for. In the present-day frame, the elderly Jacob, forgotten by much of his family and stuck in a nursing home, slips away to a circus that has set up nearby and persuades its owner to take him on to sell tickets, choosing to spend his last days back in the world he loved. The novel closes on his contentment at having found, even in old age, one more chance for the life he never stopped longing for.
