Eldridge Tyler takes up a concealed position beside an old barn after four associates ask him to stop an approaching stranger. That stranger is Jack Reacher, a former military policeman hitchhiking toward Virginia despite severe strain in both arms. At a rural inn, Reacher compels an intoxicated doctor to visit a patient with persistent bleeding. The patient is Eleanor Duncan, whose husband Seth has abused her for years. Two outside enforcers have also beaten her because of trouble involving Duncan Transportation. Reacher survives an attack ordered by Jacob Duncan and learns that the family's delayed cargo is moving through a criminal distribution chain involving Rossi, Safir, and Mahmeini. Eleanor remains with the Duncans in hopes of outlasting them. Reacher postpones his departure to examine the twenty-five-year-old disappearance of eight-year-old Margaret Coe. The archived investigation shows that the Duncan properties were thoroughly searched and that the family had a corroborated alibi. It also supports the innocence of Margaret's father, Arthur Coe. Reacher nevertheless suspects that the missing answer lies outside what investigators recorded. Meanwhile, six armed men from competing links in the supply chain converge on the area to kill him, and the Duncans pressure the doctor into claiming that Reacher has left.

Roberto Cassano and Angelo Mancini, working for Rossi, kill Safir's two men. Mahmeini's remaining operative steals the Cadillac, finds Sepehr's body, and wrongly concludes that the Duncans are eliminating every intermediary. After he tries to force the family into selling directly to Mahmeini, Cassano and Mancini kill him and align themselves more closely with the Duncans. Seth and his men capture Reacher, break his nose, and imprison him in the doctor's reinforced basement. The doctor, his wife, and Dorothy Coe are also coerced. Reacher cuts the power, lures two guards into the basement, incapacitates them, and frees the other captives. Though injured, he arms himself with their shotgun and resumes control of the doctor's house. At dawn, the unidentified shipment leaves a Canadian transfer area in a white van.
