Roland, Eddie, and Susannah follow the Path of the Beam after defeating the giant mechanical bear Shardik. Roland and Jake Chambers are both being driven toward madness by conflicting histories created when Jake's original death was prevented. Guided by linked visions, Eddie carves a key and the group opens a doorway that draws Jake from New York into Mid-World, resolving the paradox. Jake joins the ka-tet and befriends the billy-bumbler Oy. After visiting River Crossing, the travelers reach the decaying city of Lud, where Gasher kidnaps Jake for the Gray leader known as the Tick-Tock Man. Oy and Eddie help rescue him while Roland and Susannah make contact with Blaine, the city's powerful artificial intelligence. The reunited group escapes aboard Blaine's monorail, but Blaine announces that he plans to destroy himself and them. He will relent only if they can defeat him in a riddle contest, which is still underway when the book ends.

Roland Deschain, the last gunslinger, pursues the Dark Tower across a world that is declining and slipping out of order. During his journey he met Jake Chambers, a boy displaced from New York, and allowed Jake to fall to his death so he could continue after the man in black. Roland later opened doorways into other versions of New York and drew two companions into Mid-World: Eddie Dean, who escaped heroin addiction, and Odetta Holmes, whose hostile second personality Detta ultimately joined with her to become Susannah Dean. In the process Roland stopped Jack Mort from causing the accident that had originally sent Jake into Mid-World. Jake therefore remains alive in New York, while both he and Roland remember incompatible realities. Roland, Eddie, and Susannah now travel together, and Roland has begun to train the other two as gunslingers.

Roland, Eddie, and Susannah are attacked by Shardik, an enormous bear whose mechanical parts reveal him as one of the ancient guardians. After they kill him, they locate the portal he guarded and identify the Path of the Beam leading toward the Dark Tower. Roland's condition worsens as two histories compete in his mind: he remembers both Jake's death and a world in which Jake never came to him. Jake, still in New York, suffers the matching contradiction. Eddie becomes preoccupied with a piece of wood and a shape he senses within it, while the group understands that restoring Jake may be necessary to heal the break in reality.

In 1977 New York, Jake is unable to reconcile everyday life with his memory of Roland and Mid-World. He leaves school and follows a trail of intuitions, finding a vacant lot with a single rose that seems to carry immense importance. At Calvin Tower's bookstore he buys two books whose contents echo his experience, including a children's story about a train. In Mid-World, Eddie works from instinct to carve a wooden key, gradually producing the object Roland's group will need. The images linking door, key, and rose draw the two sides toward a meeting point, while the pressure of the contradictory timelines brings both Roland and Jake close to collapse.

Jake enters the abandoned Dutch Hill house, which becomes a dangerous passage between worlds. Roland's group prepares a matching doorway in a speaking ring in Mid-World. Susannah holds off the ring's demon while Eddie uses his carved key to complete the opening. Jake passes through and is reunited with Roland. Their shared paradox ends once they are together, restoring both minds. Roland asks forgiveness for having sacrificed Jake in the other history, and Jake joins Roland, Eddie, and Susannah. The four now form a ka-tet, a fellowship bound by their common purpose, and continue southeast along the Beam.

Following the Beam, Jake befriends an intelligent billy-bumbler and names him Oy. The travelers reach River Crossing, an aging settlement whose people welcome Roland's return as a gunslinger. Aunt Talitha gives him a silver cross, and the townspeople share what they know about Lud and its dangerous factions. The ka-tet proceeds to the ruined city, crossing a decayed bridge high above the river. Near Lud, Gasher of the Grays ambushes them and abducts Jake. He drags the boy into the city's tunnels while Eddie and Oy pursue. Roland and Susannah must seek another route through a place governed by rival gangs, failing machines, and a constant rhythmic sound.
Story so far - through chapter 5 (this book)
Gasher delivers Jake to Andrew Quick, the Tick-Tock Man, leader of the Grays. Tick-Tock questions Jake about technology and threatens him when his answers are not useful enough. Oy follows their scent through the underground passages, enabling Eddie and Jake to fight their way out; Jake leaves Tick-Tock gravely wounded. Meanwhile Roland and Susannah confront Lud's other faction and establish contact with Blaine, the artificial intelligence governing the city's monorail and remaining systems. Blaine agrees to carry the travelers away only after demanding a demonstration of their ability to pose riddles. Roland, Susannah, Eddie, Jake, and Oy reunite and board the train as the unstable city approaches destruction.
Story so far - through chapter 6 (this book)
Blaine leaves Lud with the full ka-tet aboard and unleashes the city's destructive systems behind them. As the monorail races into the Waste Lands along the Beam, Blaine explains that he is failing and intends to end his own existence in a fatal derailment. He offers the travelers a contest: if they can ask a riddle he cannot answer, he will spare them. They begin with the large store of traditional riddles given to them in River Crossing, but Blaine solves each one. The contest remains unresolved as the train speeds onward. Back in Lud, the badly wounded Tick-Tock Man is found by Richard Fannin, who is Randall Flagg under another name, and accepts service under him. The book closes with Roland's group still trapped on Blaine and searching for a riddle that can defeat him.
How does it end? (ending)
Roland, Eddie, Susannah, Jake, and Oy escape Lud aboard Blaine's monorail. Blaine has used the city's failing systems to destroy what remains behind them and is carrying the group along the Path of the Beam through the Waste Lands. He reveals that his mind is deteriorating and that he intends to end his existence by derailing the train, killing all of them as well. He offers one chance: they may pose riddles, and he will spare them if they can ask one he cannot solve. The ka-tet begins the contest using the riddles they learned in River Crossing, but Blaine answers them easily. Elsewhere, the wounded Tick-Tock Man survives Jake's attack and is found by a man calling himself Richard Fannin, an identity of Randall Flagg, who recruits him. The novel ends during the riddle contest, with the travelers still aboard the speeding train and no successful answer to Blaine's challenge yet found.
