Eighteen months after Simon Kelleher's death, an anonymous account revives his methods at Bayview High with a game of Truth or Dare run by text: each target is named in public, offered a choice, and exposed if they refuse. Phoebe Lawton is one of the first casualties. Maeve Rojas, whose sister was one of the Bayview Four and who is hiding the possibility that her cancer has returned, and her friend Knox Myers set out to identify the account before it reaches them. The school treats the game as a prank until a dare kills the athlete Brandon Weber and the town faces a second homicide investigation. Working from what the account knew and when it could have known it, the three of them establish that the game was started by a student imitating Simon and then taken over by Phoebe's older sister Emma, who arranged Brandon's death and used the game to cover it. Emma is arrested, Maeve stops hiding her health from her family, and Bayview learns that anyone can pick up where Simon left off.

Eighteen months earlier, five Bayview High students were sent to detention and one of them, Simon Kelleher, died of a deliberately induced allergic reaction. Simon ran About That, a gossip app that traded in classmates' secrets, and each of the four survivors was the subject of a post he had scheduled for the following day. Those four, Bronwyn Rojas, Addy Prentiss, Nate Macauley and Cooper Clay, became national news as the Bayview Four while an anonymous account published the secrets Simon had collected: Bronwyn's cheating on an exam, Addy's infidelity, Nate's dealing, Cooper's relationship with a young man. Nate was arrested and charged. Investigating on their own, with help from Bronwyn's younger sister Maeve, they established that Simon had staged his own death to frame them and to be remembered for it, with help from Addy's boyfriend Jake Riordan, who attacked Addy when she confronted him and was arrested. The four were cleared. Bronwyn is now at Yale and still with Nate, Cooper is playing professional baseball and is open about his relationship with Kris, and Addy lives with her sister Ashton. Bayview has never stopped being the town where it happened.

An anonymous account begins running a game of Truth or Dare at Bayview High. Targets are named where the whole school can see it and given a choice: answer a truth the account already knows, or complete a dare. Refusing means the truth is published anyway. The material is accurate and personal, which tells everyone that whoever is behind it has been collecting information the way Simon Kelleher once did, and Bayview reacts to the echo immediately. Phoebe Lawton is among the first named and refuses, and the account makes good on the threat, which costs her at school and at home. The administration treats the game as a prank to be suppressed and the police have nothing they can act on, so the people taking it seriously are the students it is aimed at. Maeve Rojas and Knox Myers start trying to work out who is running the account, partly because they remember how the last one ended and partly because each of them has something they would rather not see published.

The game keeps going and the school keeps watching. More students take the dare rather than risk the truth, which is what preserves the account's audience and its power, and the dares get harder. Phoebe lives with the aftermath of her own exposure, including at home, where her older sister Emma has held the family together since their father died and is assumed by everyone to be coping. Maeve has been hiding the possibility that her cancer has come back and knows precisely what the account could do with that, so she says nothing to Bronwyn, to her parents or to Knox. Knox, interning at Until Proven, has access to Eli Kleinfelter and to adults who would act if anyone could give them a name, and nobody can. Nate Macauley and Addy Prentiss, who know exactly where this leads, are drawn back in through Maeve. The account has not run out of names.

A dare ends in the death of Brandon Weber. What the school had treated as an ugly game becomes a homicide investigation, and a town that has already been through one student death and the coverage that followed braces for a second. The account does not stop; the game continues while the police work, which keeps suspicion moving around the school and makes it impossible to tell targets from participants. Maeve, Knox and Phoebe, each far closer to the game than the adults realize, begin working through what the account knew about each person and when it could have learned it, on the reasoning that its knowledge is the only real evidence anyone has. Maeve's health can no longer be kept from her family. The Bayview Four are pulled back into the story by a town that assumes anything of this kind must connect to them.

Bayview ends with a second dead student and no confidence that the pattern Simon started will not be picked up again.

Maeve has stopped hiding her health from her parents, from Bronwyn and from Knox, and she and Luis Santos have become close. Knox has stopped treating himself as the least important person in the room. Bayview has a second dead student and the knowledge that the pattern Simon started can be picked up by anyone who wants it.
