In 1945, former wartime nurse Claire Randall is on a second honeymoon in the Scottish Highlands with her historian husband, Frank. Exploring an ancient circle of standing stones, she is hurled two centuries into the past, to 1743. There she is menaced by a cruel English officer who is the double of Frank, his own ancestor, and taken in by clansmen of the MacKenzies, who suspect she may be a spy. Stranded and unable to return home, Claire uses her medical skill to survive and grows close to a brave young Highlander, Jamie Fraser. To protect her from English jurisdiction, she is compelled to marry Jamie, and their marriage of convenience becomes a deep and passionate love. Caught between two eras and two men, and hunted by the sadistic Captain Randall, Claire chooses to stay in the past with Jamie. Their happiness is nearly destroyed when Jamie is captured and brutally tortured by Randall, but Claire mounts a desperate rescue and nurses him back from the brink. The book ends with the couple safe for the moment in France and Claire pregnant with Jamie's child.

In 1945, Claire Randall, a former army nurse, is holidaying in the Scottish Highlands with her husband Frank, a historian tracing his ancestors. Visiting an ancient ring of standing stones, Claire touches one and is suddenly transported back to the year 1743. Disoriented in a wild and dangerous era, she is accosted by a British army officer, Captain Jonathan Randall, who is the startling image of her own husband and his distant forebear. She is then seized by a band of Highland clansmen of the MacKenzies, among them a wounded young man she tends with her nursing skills, and carried off to their stronghold, Castle Leoch, where she is regarded with deep suspicion as a possible English spy and finds herself unable to explain who or what she really is.

At Castle Leoch, Claire is taken in and set to work as a healer, her medical knowledge earning her a place while her strange manner keeps the clan wary. She meets the shrewd, ailing laird Colum MacKenzie and his forceful war-chief brother Dougal, who cannot decide whether she is friend, spy, or something stranger, and she grows closer to the good-natured young warrior, Jamie Fraser, learning the burdens of his outlawed past. Kept as a guarded guest, Claire secretly plots how she might return to the standing stones and her own time, all the while adjusting to the harsh, precarious life of the eighteenth-century Highlands and the clan politics swirling around her.

Claire travels with Dougal and a party of clansmen as they ride the countryside collecting the laird's rents, and she glimpses the undercurrents of Scottish resistance to English rule. The threat posed by Captain Randall, who has legal power over Claire as an Englishwoman, grows more acute. To place her beyond his reach under the law, Dougal contrives that she marry a Scot, and she is wed to Jamie Fraser in a hasty arrangement meant purely for her protection. What begins as a marriage of necessity quickly deepens, and Claire finds herself drawn to Jamie far more than she expected, even as her heart remains torn between the husband she left behind and the life taking shape around her.

Claire and Jamie's marriage grows into a genuine and passionate bond, though Claire never abandons the hope of returning to her own time. Seizing a chance, she slips away toward the standing stones, only to be captured by Captain Randall and brought to Fort William, where his cruelty is on full display. Jamie stages a bold and reckless rescue, breaking in to save her and defying the English officer directly. The escape deepens the danger surrounding them and the enmity between Jamie and Randall, and it forces Claire to weigh, more seriously than ever, where and with whom she truly belongs as the couple flee back toward safety.

Suspicion of witchcraft falls on Claire, and she is seized along with the herb-wise Geillis Duncan and put on trial, in mortal danger until Jamie arrives to save her. In the aftermath Geillis reveals a secret that shakes Claire: she too came from another century through the stones, a committed Jacobite who came to influence history. At the standing stones Claire finally tells Jamie the truth about where and when she is from, and he gives her the choice to return to her old life; she chooses to stay with him. Jamie then brings Claire home to his family estate, Lallybroch, where he reclaims his place as laird and she meets his fierce sister Jenny. For a time they build a life there, but the shadow of the English and of Randall continues to loom over them.

Their fragile peace is torn apart when Jamie is captured and imprisoned at Wentworth Prison under sentence of death, where Captain Randall subjects him to brutal torture and sexual assault that nearly destroys him body and soul. Claire refuses to give him up for lost and, with the help of Jamie's kin, carries out a desperate rescue that frees him. The couple escapes across the sea to a French abbey, where Claire labors to heal Jamie's shattered body and mind, at last driving him through a harrowing confrontation with his trauma so that he can begin to recover. As the story ends, Jamie is slowly mending, the two are more deeply bound than ever by all they have endured, and Claire discovers that she is pregnant, carrying their child into an uncertain future amid the gathering intrigues of Jacobite France.
